Athletic Screening Night
Wednesday, June 6, 2012 Clovis East High School (West) Gym TIME : 6:00 pm – 8:00 pm COST : $30.00 for Health Screening
___________________________________________________________________ All intermediate and high school athletes need to have a health screening/physical every year. The Reagan Educational Center is offering this evening as a service to our parents and athletes. Save the hassle of having to make an appointment at a doctor’s office or health clinic! There will be doctors and nurses on duty the night of the physicals to screen athletes. All Reyburn and Clovis East Athletic Eligibility Forms will be on hand that evening. Parents can fill out the forms, turn them in, and be done with it for the rest of the year! Bring proof of insurance or purchase school insurance that night. ____________________________________________________________________________
